There are two purposes of the Rosacea-Ltd III Progress Follow Up Form: 1) PRIOR to the start of Rosacea-Ltd III as you might have questions or concerns of your future use of Rosacea-Ltd III or existing problem prior to the start of Rosacea-Ltd III. And 2) AFTER the beginning of the application of Rosacea-Ltd III that you may have a question of how to gain more improvement.

We wish to further assist you to gain the progress that you desire. So we need to know what you are doing and not doing. If you have NOT started the application of Rosacea-Ltd III, then please tell us in the comment section. If you have started the application of Rosacea-Ltd III, please give us the date that you started the application likewise in the comment section along with any other comments. We will realize that the "measurement of the improvement scale from "1 to 10" will NOT need to be completed unless you are "applying" Rosacea-Ltd III at the present time.

To assist you in improving the appearance and condition of your rosacea, acne or seborrheic-dermatitis skin condition, please complete "all blanks" below.
